When treating maxillary posterior cases, 64% of patients lack enough bone for implants. In this case, missing from the 1st premolar to the 2nd molar, the molar sites required sinus grafting.

We analyzed 5 options:
1️⃣ Single Premolar: No graft, but limited functional surface.
2️⃣ 2 Implants/3-Unit Bridge: Full function, but requires sinus grafting (higher cost/risk).
3️⃣ Tilted Implant/Bar: Graftless, but custom bars increase lab costs significantly.
4️⃣ All-on-4: Maximum function, but overkill if healthy natural teeth remain.
5️⃣ The "Balanced" Option: 1 implant at the premolar site restored with a molar crown. This avoids grafting, controls costs, and provides an optimized functional outcome.
