Avoiding the "Devil in the Details" of Guided Surgery
For esthetic and restorative-driven implant planning, we use the gingival zenith as our primary guide—even if it means planning adjacent implants at different depths/trajetories to achieve the best result.
While the "master plan" may look perfect, the transition to surgical guide design is where the "devil in the details" often hides. Take this 2-unit anterior case as an example:
If we don’t pay close attention during the design phase, we might miss a critical risk: sleeve collision between the guided instruments and the sleeves. Because the space is narrow and the sleeves sit at different heights and trajectories, the guided kit can actually get "caught" on the adjacent sleeve during surgery. In this case, one sleeve was 0.4mm higher than the other—a tiny margin that can cause significant deviation and compromise the esthetic outcome.
Once you identify the problem, the fix is simple. One of the most effective ways to address this is to design two separate guides. This eliminates sleeve collision while allowing for a fully-guided procedure.
